Insurance Information

When we have a contract with an insurance company, we will bill that insurance as a courtesy to you. It is important that you know the provisions of your own insurance. You will be expected to pay all co-payments or deductibles at the time of service.
           
We currently have a contract with the following insurances:

            Aetna
            Anthem Pathway PPO
            Pathway X PPO
            Beechstreet
            Blue Cross
            Blue Shield
            CCN
            Cigna
            Coastal
            CHP (third party administrator)
            First Health
            Medicare
            Pacific Health Alliance
            Tricare
            Unitedhealthcare

If you have no insurance or you have an insurance in which we do not have a contract, you will be expected to pay for all costs associated with your visit at the time of service.

Self-pay patients or patients who must pay at the time of service receive a 20% cost savings at the time they pay the bill on the day of service. Because we do not bill your insurance, nor do we have to follow-up on payments from insurance companies, we pass our cost-savings on to you.

Billing your own insurance?
If you pay your bill at the time of service, but would then like to bill your insurance yourself to reimburse you the costs of the visit, please request a copy of the bill (HCFA form) from the registrar at the front desk. The HCFA form is the billing form you will need to send to your insurance company. The HCFA form has all the codes and numbers the insurance company will need to reimburse you! A blank HCFA form is also available by clicking on the "back" button or going to the "Billing" page: see "Download the HCFA Form" under the Insurance section.

Secondary Insurance:
When you have two insurances, we will bill your secondary (second) insurance only once as a courtesy. However, if your second insurance company does not pay MBUC within 30 days, the clinic will then bill you for the amount directly. We do not follow up with secondary insurance companies for payment beyond one attempt. We do not bill Railroad Medicare under any circumstance.
